| The State of California
Department of Consumer Affairs regulates the general provisions and requirements
of the Structural Pest Control Act. The information listed has been
obtained from the Consumer Affairs Structural Pest Control Act issued by
Mary Lynn Ferreira, Registrar. The department is located at 1422
Howe Avenue Suite 3, Sacramento, California.
The pest control
company is required to retain for three years all field reports from which
a verbal or written estimate of, or solutions for, work are made.
A written inspection report shall be prepared and delivered to the person
requesting the inspection or to the designated agent. A copy of each
report must be filed with the board at the time the report is delivered,
or not later than five working days after the date the inspection is made.
The following information must be set forth in the report: |

| 1 . The date of
inspection and the name of the licensee making the inspection.
2. The name and address
of the person ordering the report.
3. The name and address
of any person who is a party in interest to whom the board is to send certified
copies of inspection reports and completion notices.
4. The address or
location of the property.
5. A general description
of the building or premises inspected.
6. A foundation diagram
or sketch of the structure including the approximate location of any infested
or infected areas that exist.
7. Information regarding:
a. Substructure
b. Stall Shower
c. Foundation,
walls, and footings
d. Porches,
steps
e. Ventilation
f. Abutments
g. Attic Spaces
h. Garages
i. Decks,
Patios
j. Other
Also identified are
conditions usually deemed likely to lead to infestation or infection, such
as earth contacts, excessive cellulose debris, faulty grade levels, excessive
moisture conditions, and insufficient ventilation.
8. Indication or
description of any areas normally subject to damage which are inaccessible
or for other reason not inspected.
9. Recommendation
for correction measures.
10. Information regarding
the pesticide or pesticides to be used for control of infection or infestation
found.
11. The inspection
report shall clearly disclose that if requested, a reinspection of the
structure will be performed if an estimate was given.
12. Each copy of
the inspection report filed with the board must have affixed a stamp issued
by the board. |
Upon obtaining an inspection
report disclosing no infestation or infection the following certification
must be listed on the report or Notice of Work Completed/Not Completed
form:
"This is to certify
that the above property was inspected on __(DATE) In accordance,
with the Structural Pest Control Act and rules and regulations adopted
pursuant thereto and that no evidence of active infestation or infection
was found."
When the inspection
report prepared discloses infestation or infection, and the notice of work
completed prepared indicates that all recommendations have been completed,
the following certification must be listed:
"This is to certify
that the property described herein is now free of evidence of active infestation
or infection."
In compliance with
the above referenced rules and regulations established by the Structural
Pest Control Act along with specific guidelines established by the Federal
Housing Administration (FHA), Veterans Administration (VA), Federal Home
Loan Mortgage Corporation (FHLMC), Federal National Mortgage Association
(FNMA), and CP Financial Services, an outline of suggested Guidelines
is being provided for your reference.
